Form Wizard: Add a Form

1.  Click on the Form Wizard menu.  The Add Form page displays the following fields:


Form Title (Required) - indicates the name or title of the form.

Form Header - includes additional information, such as form description
(Note:  The content entered here will be used for the keyword search.) 

Return URL - enter the URL that users will be redirected to after form submission.  The default value is "index.cfm".

Submission Message - Displays a pop-up message that confirms submission of the form. 

Email Submission To (Required) - This applies only for forms submitted from the web site.  Enter the Email Address of the recipient for the submitted form data.  Multiple email addresses must be separated by a comma (",").
(Note:  Data entered from the Administrator interface will not be sent via Email.)

Form Status - Select either Active (approved for publishing) and/or Public (accessible to all web site users)
(Note:  If Public status is not marked, the form will only be only be accessible to registered web site users.)

Form Options - Additional options for forms creation:

Allow Submission
- A check mark indicates that the form can be submitted.

Allow View
- A check mark indicates that users can view submitted results.

Allow Printing - A check mark indicates that the form can be printed.

Save Into Database - A check mark indicates that submitted results will be saved into the database.  Note:  If this option is not selected, you will not be able to view the submitted data.  Data will be sent to the email addresses entered in the Email Submission To field.

Unique Submissions
- A check mark indicates that the system will do the necessary verification to ensure that the user does not submit the form repeatedly.

Use Compact Field Layout– This applies an optimal layout for radio button and check box fields.

2.  Click the Add button.  The Form Wizard page displays.  This is where you add the form content (see help topic Add Form Fields).
