Form Wizard: Overview
The Form Wizard module provides the capability to create forms without the need for programming or HTML skills. Forms can be used to gather data from web site users and then saved into a database for future use and/or emailed to a recipient. Saved data can be exported to a CSV file for use with spreadsheet programs or other databases. |

Form Wizard: View Forms
1. Click the Form Wizard menu. The Forms View page displays the following information:
Form title - the name of the form Active flag - Yes indicates that the form is approved to be published on the web site Public flag - Yes indicates that the form can be viewed by all web site users, both members and non-members
Save data - Yes indicates that the form data will be saved in the database 2. To view the form details (header, form fields, status, options, ...), click on the form Title. |

Form Wizard: Add a Form
1. Click on the Form Wizard menu. The Add Form page displays the following fields: Form Title (Required) - indicates the name or title of the form. Form Header - includes additional information, such as form description Return URL - enter the URL that users will be redirected to after form submission. The default value is "index.cfm".
Submission Message - Displays a pop-up message that confirms submission of the form. Email Submission To (Required) - This applies only for forms submitted from the web site. Enter the Email Address of the recipient for the submitted form data. Multiple email addresses must be separated by a comma (","). (Note: Data entered from the Administrator interface will not be sent via Email.) Form Status - Select either Active (approved for publishing) and/or Public (accessible to all web site users) (Note: If Public status is not marked, the form will only be only be accessible to registered web site users.) Form Options - Additional options for forms creation: 2. Click the Add button. The Form Wizard page displays. This is where you add the form content (see help topic Add Form Fields).

Form Wizard: Add Form Fields
1. First, add the form (see help topic Add a Form). The Form Wizard page should be displayed.
2. In the Form Header box, enter form description information or instructions for completing/submitting the form. 3. Click the Edit Fields button. 4. Click the Add Field button. 5. Select the Field Type (Required). 7. Enter the Field Label (Required). 8. Select the form field properties (e.g. Maximum Characters, Character Width, Initial Value, Required Field, etc...). 9. Click the Add button. The form field is added to the form. 10. To continue adding fields to the form, click the Add Field button and repeat the steps above. |

Form Wizard: Edit a Form Field
1. Click on the Form Wizard menu.
2. Click on the form name for the form you want to edit. 3. Click the Edit Fields button. . 4. Under the field you want to edit, click Edit. 5. Make any changes to the field properties then click the Update button. 6. When the "Information Updated" pop-up message box displays, click the OK button. |

Form Wizard: Move a Form Field
1. Click on the Form Wizard menu.
2. Select the form you want to edit. 3. Click the Edit Fields button. 4. Under the form field name you want to move, click the Move Up link or click the Move down link to position the field on the form. |

Form Wizard: Delete a Form Field
1. Click on the Form Wizard menu.
2. Click on the form name that contains the field you want to delete. 3. Click the Edit Fields button. 4. Under the form field name which you want to delete, click Delete. 5. To confirm the deletion, click OK. To cancel the deletion, click Cancel. |

Form Wizard: Export Form Data
1. Click on the Form Wizard menu.
2. In the Form Data section, select the form name from the pull down list. 3. Click the View Data button. 4. Click the Export to CSV button. 5. Select what you want to export (e.g., Form title, Form header, field names, ...). 6. Click the Export button. 7. Click the Save button to save the exported data to a specific disk location (or click the Open button to open/view the exported file). |

Form Wizard: Find a Form
1. Click on the Form Wizard menu.
2. Click in the search box next to the Find button and type the name of the form (or part of it) that you are searching for. 3. Click the Find button. The search results are displayed. 4. To view the form details, click on the form name. |

Form Wizard: Copy a Form
1. Click on the Form Wizard menu.
2. Select the form you want to copy. 3. Click the Copy button. 4. To confirm, click OK. A copy of the form is created. Example: A copy of the form "TEST" will be created as "TEST Copy". |

Form Wizard: Delete a Form
1. Click on the Form Wizard menu.
2. Select the form(s) you want to delete. 3. Click the Delete button. 4. To confirm the deletion, click OK. To cancel the deletion, click Cancel. |
